This form is intended for users of Lenvica HRMS to make functional, behaviour, or design change requests on Lenvica HRMS. If you are a registered user of Lenvica HRMS, and have a change request, please fill out the first part of the form and email it as an attachment to support@lenvica.com

Instructions

1. The Change Request Form is a Microsoft Word file document that should be downloaded and then completed by the requester.

2. Use a seperate form for each request for change.

3. Use attachments whenever necessary. Attachments can be in the form of documents or images. Screenshots should be included whenever possible.

4. Provide comprehensive details on the change.

5. Send the completed document and attachments only in electronic format.

6. Depending on the complexity of the request Lenvica might take 2 or more working days to respond on the change request.

7. An approval from both Lenvica and the client is required before any work on the task is executed.
